The 2012 Annual Enrollment period is November 17 – December 2, 2011. Changes made to your benefits during this time will become effective January 1, 2012.

In addition, you may also call the Frontier Benefits Service Center at 1-877-FTR – 2770, M-F, 9 a.m. to 6 p.m. Eastern, to enroll for your 2012 benefits.
Once you complete your 2012 enrollment, be sure to print a copy of your Confirmation Statement. The Confirmation Statement is not being mailed to you. Once the 2012 Annual Enrollment period ends on December 2, 2011, you cannot change your 2012 benefit elections unless you have a qualified change in status as determined by the IRS during the 2012 plan year.
Please logon to www.yourbenefitsresources.com/frontier, prior to November 17th, the first day of the Annual Enrollment window, to create a user ID and password if you don’t have one. You may contact the Frontier Benefits Service Center at 1-877-FTR-2770 should you need help setting up your account.
